Working with the three acute trusts within the Network - University Hospital Coventry and Warwickshire, South Warwick Hospital and George Eliot Hospital - we have supported the development of an integrated care pathway for Stroke admissions that will follow the patient along the care pathway.

Accelerated Improvement project (ASI)
The Accelerated Stroke Improvement project supported the delivery of services included in the ASI metrics. ASI Metrics
The metric covers 9 areas of the whole Stroke patient pathway from management of Atrial Fibrillation through to annual follow-ups. Specific projects relating to ASI metrics 6 - 9 have been supported by the Network:
ASI 6 - Timely access to psychological support - this project has developed a single assesment process and model which is to be adapted across the Arden health economy through the Pyschological Consultant Group which has representatives from all of the local Trusts and IAPT.

ASI 7 - Joint health and social care management - a single assement form has been developed and trialed with South Warwick Foundation Trust Stroke unit and is now being introduced by University Hospital Coventry and Warwickshire and George Eliot Hospital.

ASI 8 - Assessment and review (6 weeks, 6 months and annual reviews) and ASI 9 - (Access to and availability of ESD services) - these projects are either part of, or subject to the outcome of the Midlands and East Strategic Stroke Review so will be implemented at a later date.
